A useful learning conversation starts with a specific question: what does a written standard mean in the setting where you work? These prompts help a team organise that conversation and identify what needs checking.

Choose one question and its source

Bring the current document you want to discuss and identify the section, version and local context. Keep what the document says separate from an interpretation, a personal experience or a proposed change. If the wording or applicability is uncertain, record that uncertainty for the appropriate clinical or governance lead.

Hear the different perspectives

Ask how the issue looks to someone delivering care, a learner, a trainer and the organisation responsible for the service. Consider the patient perspective without sharing identifiable patient information. Differences between these accounts can become questions to investigate; they are not, by themselves, evidence of a failure or an explanation of its cause.

Three prompts for the discussion

• Where do you need greater clarity about applying the written standard in your setting?
• What would help you distinguish a learning need from a question about resources, workflow or documentation?
• Who needs to check the interpretation, and what evidence would help them decide the next step?

Leave with a question someone can act on

Record the question, the source, what remains uncertain, an owner and a review point. A discussion does not itself authorise a change to local policy or clinical practice. Keep those decisions within the relevant clinical and organisational process.
